I consent to the dissemination (publication) of my image in the form of photos and videos from the implementation of the NEXT_LEVEL Project (hereinafter referred to as the ‘Project’) and the processing of my basic personal data (in particular name and surname, name of the university/ institute, information about the participation and results of the Project) by the Institute of Human Genetics of the Polish Academy of Sciences (Poland), University of Groningen, University Medical Center Groningen (The Netherlands), Ghent University (Belgium), Ulm University (Germany) (hereinafter referred to as ‘Partners’) in order to promote the Project, Partners, and disseminate project results. The consent is free, it is not limited in quantity, time or territory; covers all forms of publication, via any medium, in particular on the websites of the Partners, the website nextlevel-h2020.eu, other websites, social media of the Partners, radio and television. The image can be used for all kinds of electronic image processing, framing and composition, with no obligation to accept the final product, but not inoffensive or generally considered unethical forms. This consent covers the recording, processing and duplication of photos/videos in electronic and printed form. I have read the content of the Partners’ information clauses regarding the processing of personal data. I have been informed that the consent is voluntary but necessary for participation in the Project, and that I have the right to withdraw my consent at any time, and the withdrawal of consent does not affect the lawfulness of the processing that was carried out on its basis prior to its withdrawal.
